After a horrific car accident, Anna (Ricci) wakes up to find the local funeral director Eliot Deacon (Neeson) preparing her for her funeral. Confused, terrified and feeling still very much alive, Anna doesn't believe she's dead, despite the funeral director's reassurances she's merely in transition to the afterlife. Eliot convinces her he has the ability to communicate with the dead and is the only one who can help her. Trapped inside the funeral home, with nobody to turn to except Eliot, Anna's forced to accept her own death. But Anna's grief-stricken boyfriend Paul (Long) can't shake the suspicion that Eliot isn't what he appears to be.

When Dallas FBI Agent Thomas Mackelway violates serial killer Raymond Starkey's civil rights during an unorthodox arrest, Starkey goes free and Mackelway is demoted to a remote branch of the agency in Albuquerque. His first day on the job, Mackelway investigates the murder of traveling salesman Harold Speck, which turns out to be the first of three seemingly random killings. Perhaps they are not random at all. The last to die is Mackelway's nemesis, Raymond Starkey. The assignment consumes him, his past mistakes haunt him, and his head throbs constantly as he tries to find the link between the victims that will lead him to their killer while the case becomes increasingly gruesome and patently personal. This does not go unnoticed by his unflappable partner Fran Kulok, who knows of Mackelway's past and the demons that afflict him. Like Mackelway, she becomes drawn into the labyrinth of chilling clues, all of which point to the enigmatic Benjamin O'Ryan. O'Ryan clearly has a connection to the murders, a connection he flaunts. Quite possibly, he may also harbor a sinister link to Mackelway.

A few incidents that happen at 11:14 p.m. in the town of Middleton are seen from five different perspectives, each of the players who generally works toward self interest, not caring about the consequence to other people, and in the process exacerbating the situations by his or her actions. Jack, in town to meet someone, is a chronic drinker and driver, he still doing so despite having his license temporarily suspended. Teens Mark, Eddie, and Tim are out for a joyride in Mark's van, they generally causing havoc all in the name of having fun, that is until something goes wrong. Frank, out for a walk with his dog, stumbles across something which makes him jump to a conclusion about his daughter, to whom he is both overprotective and overly trusting. Duffy, who works as a clerk at Lloyd's EZZZ Stop convenience store, goes to the store just as it is closing to ask his friend, the clerk on duty, Buzzy, for a favor, which may be difficult for her to grant based on her own needs, and Cheri is trying to get a few hundred dollars for an abortion from the purported "fathers", while in reality, she has another plan in mind. All their actions make for a busy night for the first responders on-duty, including Officer Hannagan, and paramedics Leon and Kevin.

After Lexi leaves home to visit Central LA, there's a terrorist attack involving chemical bombs. After the attack, her musician husband, Brad, fails to find her and reluctantly seals himself inside his house. He will have to deal with this decision in the days to come.

A soldier introduces himself to the Peterson family, claiming to be a friend of their son who died in action. After the young man is welcomed into their home, a series of accidental deaths seem to be connected to his presence.

In a parallel story, the police detective investigating the murder is having problems of her own. She is going through a messy divorce and a violent criminal who had vowed revenge some years before has escaped from prison and is after her.

Screenwriter David Sumner and his wife Amy travel in his Jaguar to her hometown, Blackwater, Mississippi. Amy's father has died and David intends to write his Stalingrad screenplay in the house. Grisly murders occur in a small town. Two FBI agents arrive, set up their cameras in three interview rooms, and set up interviews of three survivors: a girl of about nine, a foul-mouthed cop with a bandaged hand, and a young woman of about 20 who uses drugs. Each tells their story as the male FBI agent listens and watches from a separate room: the girl draws for and talks to the female agent, the local chief interviews the injured cop, and two officers interview the young woman. As they tell their stories, some of which are inaccurate and self-serving, we see what actually happened the day before. Can the agents or anyone else put the pieces together?

On a remote island, the FBI has a training program for their psychological profiling division, called "Mindhunters", used to track down serial killers. The training goes horribly wrong, however, when a group of seven young agents discover that one of them is a serial killer, and is setting about slaying the others. Can the few that are left figure out who the killer is in time?
