When the express elevators in the Millennium Building, one of New York's most famous landmarks, start to malfunction and behave in erratic ways, elevator mechanic Mark Newman is sent out to find the cause of the problems. His investigation meets unexpected resistance and not everybody seems to be happy with his involvement. After a gruesome and deadly incident, in which a blind man falls into the shaft and a security guard is decapitated, the police start an investigation. They are however not very successful. Mark is determined to find the cause. He is joined by a female reporter, Jennifer, who is looking for a juicy story. Together they try to unravel the secrets behind the mysterious behaviour of the elevator that seems to have a life of its own. When things get worse and death toll rises, The Government, fearing terrorists are involved, seal off the building. Recuperating from a broken hip sustained four months ago, wealthy Cornelia Hilyard had a portable elevator installed temporarily in the home she shares with her thirty year old single son, Malcolm Hilyard. Shortly after Malcolm leaves for the extended July 4th weekend during a summer heatwave, Cornelia gets trapped in the elevator when a short circuit occurs, the front grated elevator car hovering approximately ten feet above her living room foyer, she only having access to those few items she brought into the elevator with her: beyond those items on her person, a book, a transistor radio, her cane and a vase containing a single flower. All her efforts to get out using the elevator's emergency functions only attract the wrong elements as two associated but disparate groups break into her house initially with the sole purpose of robbing her of everything, they able to do so partly in being able to conduct their business out of her view. In the process, none of the five people involved seem to care about her whatsoever. Things change when one of the five, Randall Simpson O'Connell, the leader of the group of younger thugs, he who has spent most of his formative years in a reformatory, decides that in addition he wants to commit murder, including of Cornelia. If Cornelia is able to survive in the short term, the nature of the situation may further change due to something that Malcolm had left for her just before he left.

In Philadelphia, Detective Bowden is still grieving for his wife and son, killed in a hit-and-run five years ago. When someone jumps from a skyscraper onto a truck, Bowden is sent to investigate. Meanwhile, five strangers are trapped in an elevator in the building where the jumper committed suicide. The communication radio in the elevator is broken but the guards, Lustig and Ramirez, observe the individuals via CCTV as events unfold. Tensions run high among those trapped, so Lustig calls the police and Detective Bowden assumes the case. Without being able to contact the individuals, he tries to work out who they are, but he can only account for four of them. Time is running out for the occupants of the elevator, as Bowden realizes he has to get them out quickly.
