Sotobamura is a small village with around 1300 residents; so small the village isn't even connected to a single highway. An isolated village in which old customs, such as the burial of the dead, are still practiced. One day, the bodies of three people are found dead. Although Ozaki Toshio, the village's lone doctor, feels uncertain, he treats the deaths as a normal occurrence. However, in the days following, the villagers start to die one after the other.

An old school building rumoured to be haunted is the place where a young woman was left to die. A freshman at the school, Niija Teiichi, stumbles upon the ghost of this woman and seems to be the only one that can see her. She can't remember the events that led up to her death and hence Niija decides to help her in discovering the cause.

Follows the adventures of Koyomi Araragi, a former vampire. One day, a classmate named Hitagi Senjougahara, a loner, falls down the stairs into Koyomi's arms. Hitagi appears to weigh next to nothing, defying physics. Despite being threatened by her to keep away, Koyomi offers his help, and introduces her to Meme Oshino, a strange middle-aged man living in an abandoned building, who cured him of vampirism. Koyomi becomes gradually more involved in cases related to unusual supernatural creatures, meeting new people and discovering secrets in the people he thought he knew.

However, as Homura's omen suggests, there might be far more to becoming a magical girl than Madoka and Sayaka realize.
